I couldn't even find it on google but im gunna make an educated guess and say Randy Moss. I know he goes out on hail mary defense and he has thrown the ball on reverse passes. but just a guess.

I know Keyshawn Johnson caught an INT in his first Playoff Game when he was with the JETS but I don't think he threw one in that game so I have no clue as to who threw and caught an INT in the same game...
